Event Participation Agreement
I agree to the rules set forth by a tournament host (SPORT JIU-JITSU JAPAN FEDERATION) and to compete with the qualities of good sportsmanship.
I hereby agree to participate knowing there is a risk for an injury, death and post-traumatic syndrome that happens during or after the tournament.
I also agree not to sue the tournament host, or any related organizations for an accident, injury, death, and post-traumatic syndrome that happened during or after the tournament.
The registration fee is non-refundable and non-transferable.
Athletes who are alone in their bracket are eligible to receive a full refund of the registration fee if they contact the tournament host and request to withdraw their name from the competition prior to the end of the appointed event Refund Deadline.
I have read and understood the above statement and I, hereby, agree to participate in the tournament at my own risk. I give up any rights to the tournament host for copyrights related to the tournament. I also agree to the public display of any personal information through brochures, official results, etc.
I hereby declare that all the information in the registration is true and correct.
FOR PARENTS/GUARDIANS OF PARTICIPANTS OF MINORITY AGE (UNDER THE AGE OF 20 AT TIME OF REGISTRATION)
As a parent/guardian with legal responsibility for this participant, I certify that I have read the above statements and agree to his/her participation in the tournament.
